If you choose Zoe, both she and Mia die. If you choose Mia, not only does she live and escape, but Zoe’s story is set up for the End of Zoe DLC. In End of Zoe, in spite of what the name suggests, Zoe lives and is saved by her heretofore-unmentioned Uncle Joe.
Mia’s lifeboat section is faster and not doing the fight saves time. Definitely saving Mia, that is much faster because you skip the scene with Zoe and you also skip the fight, too.
Ending 2 – If you choose to give the Serum to Zoe, Ethan will awkwardly run off with Zoe in the boat leaving Mia behind to fend for herself. As the pair reach the Wrecked Ship, Eveline will kill Zoe, and Ethan will still be captured.
If Ethan chooses to cure Zoe Baker at the pier, then Zoe would get on the boat with him while Mia just tells them to go. However, when Eveline stops the boat and capsizes it, she calcifies Zoe and kills her as Ethan is knocked unconscious.
Resident Evil 7 doesn’t have much of a choice system. The major decision to be made in the game is whether to cure Mia or Zoe. If you give the serum to Mia, Zoe will simply disappear for the rest of the game and Mia will live until the end of the game.
To get the good ending, you’ll want to cure Mia. By saving her, the two will leave on the boat. You’ll take control of Mia during the ship stage, and when Ethan is hauled out of Eveline’s cocoon, Mia will hand him the tissue sample and ask him to go on ahead to kill her.

One of many mutant victims of biological weaponry, Winters was infected with a weaponised species of “Mold” during the 2017 Dulvey incident. He and his wife Mia were rescued by the Bioterrorism Security Assessment Alliance, and were placed under witness protection in Eastern Europe.
It turns out that the actual Ethan Winters died all the way back in Resident Evil 7, after succumbing to a punch and kick by Jack Baker. However, his exposure to the mold gave him incredible regenerative powers.
